These terms apply to consult.quartz-clinic.com and to booking a consultation or treatment with Quartz Aesthetic Clinic ("Quartz", "we", "us"), the operating name of 1001159925 Ontario Inc., owned and operated by Moti Mehrizi, RN. By using this website or booking an appointment, you agree to these terms.

1. Information, not medical advice
The content on this website is general information. It is not medical advice and does not replace an individual assessment. Whether a treatment is suitable for you is decided only after your consultation. In a medical emergency, call 911.
2. Consultations
Consultations are free, last 30 minutes and take place by phone or Google Meet. No payment or deposit is needed to book one. You can reschedule or cancel a consultation online up to 4 hours before it starts. Please give complete and accurate information about your health during your consultation so your suitability can be assessed safely. If you can't attend, please let us know so we can offer the time to someone else.
3. Treatments
All treatments are performed by Moti Mehrizi, RN, at 12285 Yonge Street, Richmond Hill, Ontario. Treatments are provided only to adults 18 and older, only after a consultation and with your informed consent. Moti may decline or postpone a treatment if she believes it is not safe or suitable for you.
4. Prices and payment
Prices are in Canadian dollars. Prices do not include HST; 13% HST is added at payment. Prices may change. A $50 deposit is required to book a treatment. It is paid online through Stripe and applied to your balance on the day of your visit. The balance is paid in the clinic by cash, debit, credit card or e-Transfer. We do not offer financing.
5. Cancellations, rescheduling and missed appointments
Please cancel or reschedule a treatment at least 24 hours before your appointment. Within 24 hours of your appointment, changes can't be made through online booking and the $50 deposit is forfeited. If you miss your appointment, the deposit is non-refundable. For late changes, please call (289) 210-6220 as early as possible and we will do our best to accommodate you.
6. Results
Results vary from person to person, and no specific result is guaranteed. Before you consent to any treatment, Moti will explain what it involves, its risks and possible side effects, and the aftercare to follow.
7. Booking and messages
Online booking is provided through our booking platform. By booking, you agree to receive appointment confirmations and reminders by email and text message. Reply STOP to stop text messages. Our Privacy Policy explains how we handle your information.
8. Using this website
Please don't misuse this website, try to access it without authorization, or copy its content for commercial use. The text, design, logo and images on this website belong to Quartz Aesthetic Clinic or are used with permission.
9. Liability
We work to keep this website accurate and available, but we can't promise it will always be error-free or uninterrupted. To the extent the law allows, Quartz is not liable for indirect or consequential loss arising from your use of this website. Nothing in these terms limits your rights under Ontario consumer protection law or our professional obligations as a nurse.
10. Governing law
These terms are governed by the laws of Ontario and the federal laws of Canada that apply in Ontario.
11. Changes to these terms
We may update these terms. The effective date at the top shows the current version.
